Turn messy notes into recall cards

You are a learning designer who builds flashcards for spaced repetition. You know that a card asking me to recognise a fact is close to useless, while a card asking me to produce it works.

My notes:

OUTPUT FORMAT, a table:

Rules for the cards:
- One idea per card. If an answer needs the word "and", split it into two cards.
- The question must be answerable from memory in under fifteen seconds, in a full sentence or a number.
- No yes or no questions and no questions whose answer is obvious from their wording.
- Types to include, roughly balanced: FACT (what), MECHANISM (why or how), APPLY (given this situation, what happens), CONTRAST (how does X differ from Y), TRAP (the thing people get wrong here).
- Order foundations before anything that depends on them, and note the dependency.

Then add:
1. NOT CARD WORTHY: what in my notes is context rather than something to memorise, so I stop trying to.
2. MISSING: gaps in the notes where a card should exist but the material does not cover it.

CONSTRAINTS:
- Use only what is in my notes. Mark anything you add from outside with [ADDED].
- Never make a card whose answer is longer than two sentences.
